Refer a Case

Habitat for Humanity Singapore collaborates with local community partners to provide intensive home rehabilitation services for vulnerable individuals and families. 

To ensure we serve those most in need with a long-term impact, Project HomeWorks accepts referrals exclusively through official social service and healthcare channels. We do not currently accept referrals from members of the public to ensure cases are managed holistically by a primary social/case worker.

We accept case referrals from professional staff of:

  • Government Agencies: MSF, AIC

  • Social Services: FSCs, SSOs

  • Healthcare: Public and community hospitals

  • Community Support: Cluster Support, AACs

 

To qualify for Project HomeWorks, the household should generally meet the following criteria:

  • Income: Low to no-income households

  • Demographics: Elderly individuals or persons with physical/mental/cognitive impairments

  • Living situation: Living alone or with an aged spouse/sibling/flatmate

  • Support system: Little to no social or family support

If you are a representative from one of the agencies listed above and wish to refer a client, please submit an application through an online referral form. Once we have received your submission, our team will follow a 5-step process.

  1. Initial discussion and assessment to review case details.

  2. Joint home visit with caseworker to assess the household's specific needs, and meet household members.

  3. Confirmation of rehabilitation works required and date of 1st Project HomeWorks session.

  4. Project HomeWorks commences, with necessary number of sessions to complete works.

  5. Update of outcome and progress after each session up to case closure by Habitat Singapore.

*Habitat Singapore prioritises cases based on urgency. High priority is given to homes with pest infestations, as these conditions can deteriorate rapidly and pose immediate health risks.
